At Oral Surgery Associates we make every effort to provide you with the finest surgical care and the most convenient financial options.
Our insurance coordinators have years of experience working with many different insurance companies. Dealing with insurance policies can be difficult and time consuming. As a courtesy, we ask that you keep us informed of any changes to your insurance. It is important that all information you provide to us be current.
Private & Group Insurance: Because many of our procedures are covered under medical and/or dental insurance, it is important to contact your carrier before the scheduled procedure to verify oral surgery benefits. As a courtesy, we will file your insurance claims for you. Upon receipt of an insurance payment, any balance due will be billed to you. If you have deposited an excessive co-payment, it will be refunded to you. Please bring your insurance card with you to the consultation so that we can expedite reimbursement.

AETNA: There are Aetna policies that use the DenteMax / Coalition of America network. When this happens, we are considered in-network for the patient. There is no identifier on the Aetna cards that reflect when a patient is in network. It is recommended that the patient contact their Aetna plan to see if we are a provider under the DenteMax network, we then will be considered in-network under medical and dental.
All other dental plans, we will work with but we are not in-network providers unless they are using the Dentemax network.
As a courtesy to our patients we will file insurance claims on your behalf, however our professional services are rendered to you and not the insurance company. It is the patients responsibility to verify in and out-of-network coverage and policy exclusions.
We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. Please remember you are responsible for all fees charged regardless of your insurance coverage.
Statements are generated monthly regardless of your insurance status. Most insurance companies will respond within four to six weeks. Please contact your insurance company if your claim is pending beyond 45 days. Any remaining balance after your insurance has paid is due in-full within 30 days. We accept Master Card, Visa, American Express, Discover, personal checks and cash.
